Coverage and Rate Information

Accepted Insurance: I accept a variety of major insurance plans, including Cigna, Optum, Aetna, Anthem, Medicare, Anthem BCBS, United Healthcare and CareSource. 

 

Out of Network/Self-Pay information: If you don't have insurance, or if I am considered "out of network" for your specific insurance plan, I offer a limited number of sliding scale appointments. This self-pay rate is not available for services that would otherwise be covered by an insurance plan where I am an in-network provider.
